Form Template Elements and Descriptions

Form Template ElementDescription
Template TypeIdentifies the kind of template, for example, a 360 review template (PM 360) or a Performance Management Review (PM Review) template. This is a read-only field that is also displayed on the Managing Form Templates page, which lists all templates in the system.
Last ModifiedDisplays a date and time stamp of the last time this form template was modified. The information is recorded automatically. This is a read-only field that cannot be edited.
Routing MapIdentifies which route map is associated to the form. You may only choose from existing route maps. A new form must be launched to see the new route map.
Default Start/End/Due Dates for Form CreationSpecifies the timeframe in which this form is due. When you create a new form, the timeframe you specified in this field is translated into a date in relation to the creation date, and that date is automatically populated on the new form. If you prefer not to use the form’s creation date as the basis for the default date, you can specify a default due date of your choice. A new form must be launched to change the default dates.
Default Due/Late NotificationDefine a number of days when an email notification is sent out after the form reaches the due date of a route step. The due date must be configured in the route map and the email notification template must be enabled for this to be functional.
Template StatusMark whether the template is enabled or disabled. Disabled templates are not visible to be launched, even if a user has permission to launch forms.
Template FlagPublic forms are transferable whenever a manager-transfer process is started. This should always be set to Public. Private is no longer a supported option.
Do Not Transfer DocumentsIf this option is enabled, this will override that document transfer and the documents will not be transferred, regardless of other settings elsewhere. Enabling this will affect live forms.
Automatic Insertion of New Manager as next document recipient if not alreadyIf this is enabled, when an employee is transferred, the new manager will be inserted as the next document recipient, if they aren’t already listed as such. Enabling this will affect live forms.
Automatic Inbox Document Transfer to New ManagerIf this is enabled, when an employee is transferred, the new manager will receive any documents that were in the old manager's Inbox. Enabling this will affect live forms. Recommended
Automatic En Route Document Transfer to New ManagerIf this is enabled, when an employee is transferred, the new manager will receive any documents that were in the old manager’s En Route folder. Enabling this will affect live forms. Recommended
Automatic Completed Document Copy to New ManagerIf this is enabled, when an employee is transferred, the manager will receive a Copy of ALL completed documents for that employee in their Completed folder. Enabling this will affect live forms. Not typically used.
Automatic Process Owner Change To New Manager For In-Progress Documents When Old Manager is Process Owner (Only for 360 Review)Revokes the original manager’s visibility to any data in the performance form (in the In-Progress inbox) after an employee has been transferred to a new manager. Recommended
Automatic Process Owner Change To New Manager For Completed Documents When Old Manager is Process Owner (Only for 360 Review)Revokes the original manager’s visibility to any data in the performance form (in the Completed inbox) after an employee has been transferred to a new manager. Not typically used.
When transferring 360 review documents from old manager`s inbox folder to new manager, do not auto decline old manager’s participant forms in evaluationWhen transferring 360 review documents from old manager`s inbox folder to a new manager, do not auto decline the old manager`s participant forms in evaluation.

Note

This option is not supported if you trigger 360 review document transfer via People Profile.
When transferring 360 review documents which are in evaluation stage, add new manager as a participant (If new manager is a removed participant, he/she will be added back.)When transferring 360 review documents that are in the evaluation stage, add the new manager as a participant. (If the new manager is a removed participant, they will be added again.) Not recommended unless it is typical that the new manager is going to be able to contribute meaningful input.

Note

This option is not supported if you trigger 360 review document transfer via People Profile.

When the system is configured to remove documents for inactive employees, apply these overrides so that the system does not remove documents for this form template

- Do not Remove Inactive Employee’s In-Progress Documents

- Do not Remove Inactive Employee’s Completed Documents

- Do not Remove Inactive Employee’s 360 Review Evaluation Documents

When an employee is set to inactive, these settings override the global setting that may remove inactive employee's documents. Enabling this will affect live forms. Not recommended unless your process requires forms for terminated (inactive) users to remain active.
Default Targets

Determines who will get this plan by default when you create forms. Choices are Self and Direct Reports (Default option), Self Only, Direct Reports Only, HR Reports Only, Self and Matrix Reports, and Matrix Reports Only.

Note

The user creating the form is called the form originator. See also Allow form creator to select anyone as the subject.
Form Timeout Alert (minutes prior to end of session)Specifies when to send a timeout alert to users. A timeout alert notifies users that their session is about to expire due to inactivity. This can help users avoid losing their work. Remember all sessions time-out in 30 minutes, so setting it to 10 means you can work for 20 minutes before the popup alert is displayed. Enabling this will affect live forms.
Hide Route Map on the FormWhen you select this option, the route map on the forms is hidden from all the participants afterward. If you deselect the option, the route map is visible to them, and they can collapse or expand it.
Keep last touched version upon Document completionWhen the form moves into the Completed route step, the form moves to the completed folder in the manager’s performance inbox. This allows the last touched or last edited copy of the form to remain visible to whoever was last to fill out ratings or comments prior to the Completed route step. Not recommended unless a person should never see updated version once they send it on.
Enable Auto Restore Deleted Form When Importing UserIn the case of employees who have been deactivated and then reactivated, this restores the most recently deleted forms for those employees when they are imported into the system. Only forms that were in an in-progress state when deleted will be restored. Completed forms will not be restored. If employee manager assignments change while employees are inactive, the Document Transfer settings will apply when employees are reactivated. This is recommended only if it is common to reinstate users and their old forms, or possibly as a one-off action at a point-in-time. Enabling this will affect live forms.
Enable Spell CheckAllows spell-checking on the entire form. Users will see the Spell Check button on the form toolbar to check comment sections, or the entire form. Enabling this will affect live forms.

Note

The Spell Check feature will reach End of Maintenance on May 17, 2024, and will be deleted on November 14, 2025. The feature is being deprecated because most modern browsers have a built-in spell check function that can be used instead.
Enable Legal ScanEnabling this option allows for Legal Scan to be used on comment fields. You may choose to upload your own library for the Legal Scan to reference when checking comment fields. Enabling this will affect live forms.
Disable Spell Check On RouteDisables spell check for the entire form when the form is routed to the next step. Enabling this will affect live forms.
Disable Legal Scan On RouteDisables legal scan for the entire form when the form is routed to the next step.
Enable Writing AssistantMakes the Writing Assistant tool available for this form. By default, the Writing Assistant is turned off and not available to users through the form. Competencies must be configured with feedback to accurately provide context-sensitive feedback with the writing assistant. Enabling this will affect live forms.
Disable Ask For Edit RoutingPrevents users from sending a form to another user outside of the defined route map (Get Feedback). Currently, the 360 Reviews – SAP Fiori Version does not support anyway this option, therefore selecting this option would not have any impact. Enabling this will affect live forms.
Allow Add Approver/EvaluatorAllows users to add users from outside the established routing map during the modify stages of the route map (Add Approver) Not particularly relevant in 360 Reviews, unless there is a need to add additional approvers not included already in the Route Map. Enabling this will affect live forms.
Disable All Toolbar Buttons (overrides all button selections below)Overrides selections to enable buttons including print, delete, info, send and open next form. Enabling this will affect live forms.
Disable Print ButtonNo longer allows users to print the form. Enabling this will affect live forms.
Disable Page Setup buttonWhen this option is not selected, users can specify which individual sections to include. When it is selected, the document is printed in its entirety.
Disable Delete buttonPrevents forms from being deleted. Deleting forms is available to only form originators and the manager of the form subject. Enabling this affects current forms that have already been launched and new forms. Enabling this will affect live forms.
Disable Form buttonPrevents users from viewing other forms about the form subject that would otherwise be viewable in a pop-up window. Enabling this will affect live forms.
Disable 360 buttonPrevents users from viewing 360 review-type forms about the form subject that would otherwise be viewable in a pop-up window. Enabling this will affect live forms.
Disable Info buttonPrevents users from viewing the document information that would otherwise be viewable in a pop-up window. Enabling this will affect live forms.
Disable Send buttonPrevents users from being able to send a form. This is often disabled if the HR team prefers to push forms through the route steps manually. Enabling this will affect live forms.
Disable Send button for completed documentsNo longer allows users to send a completed form and hides the send button during the complete step. Enabling this will affect live forms.
Disable Late NotificationSuppresses email notifications informing users when a form is late. Enabling this will affect live forms.
Disable Due NotificationSuppresses email notifications informing users when a form is due. Enabling this will affect live forms.
Disable Step Exit NotificationSuppresses email notifications informing users when a routing step has been completed. Enabling this will affect live forms.
Hide the Nonremovable checkbox on a form in the modify stageThis option allows to control whether to show or hide the checkbox on a form in the Modify stage. Previously, that checkbox always appeared on a form, regardless of whether raters are permitted to use it for editing linked competencies.
Allow form creator to select anyone as the subjectAllows users to select a form subject outside of self or direct reports. A Find User search box will appear when creating a new form using the New Document wizard available to end users under the Performance main menu option.
Allow Matrix Manager to Delete document if Delete button is availableIf the Delete button has been enabled, the Matrix Manager (EX) has the ability to delete the form. Enabling this will affect live forms.
Show Participants in Status Summary (Applicable to Anonymous 360’s Review only)Show Participants in Status Summary (Applicable to Anonymous 360 Reviews only). Enabling this will affect live forms.
Allow Re-Selection of Removed Participants to Feedback Team after 360 Review Evaluation startsAllows employees to add back Participants that have been deleted from the form. When adding back a deleted Participant, find the form in the En Route folder (My Forms page) and use the Add New Participants page to add the deleted Participant again. The deleted Participant will be shown with their original category settings. Enabling this will affect live forms.
Don’t display the subject’s Manager and HR Rep. by default, as potential participantsIf you do not want the subject’s manager or HR Rep to be visible in the default participant list, you can remove them by enabling this feature. A user would then have to manually identify these people to add them as participants.
Hide dropped users records in 360 Ad Hoc report

Note

Report synchronization is required after this configuration to ensure that dropped users in existing forms will not be displayed in the 360 ad hoc report. Please go to Company Settings and run Sync in 360 Multi-Rater Subject.

This option requires a report synchronization in the back end.

Category Weight Setting: Enable category weight normalizationEach section (competency, goal) must equal 100%, so this feature would have the system take all competencies or goals in a section and divide by 100% to average out a weight for each item.
Category Weight Setting: Hide category weight tips in Summary ViewHides category weights from raters of the form.
Category Weight Setting: Hide category weight legend in Detail ReportHides category weights from users permissioned to view Detailed 360 Report.
Participants Threshold Control: Apply Threshold On Participant Level/Item LevelParticipants Threshold Control option allows you to determine if you’d like to exclude responses at the item level (for example, by Competency) or at the document level. Options available are: Apply Threshold On: Participant Level / Item Level. Generally, the better choice is almost always Participant Level.
Enable 360 Review Drilldown Permission forEnables the selected users to drill down into the individual forms that users completed to produce the aggregate ratings. This should not be granted to the subject of the form to preserve anonymity of the raters. Enabling this will affect live forms.
Detailed 360 Report Permission Control (do not select User Form XML unless permissions have been configured by a consultant in the back end)Enables the selected users to access the Detailed 360 Report at the completion of the review cycle. The recommended option to be selected is Enable 360 Detailed Report Permission for: rather than Use Form XML unless permissions have been configured by a consultant in the back-end.
Hide Detailed 360 Review Report link unless x participants complete their formsDisables the Open Detailed 360 Report link at the completion of the process if a certain number of participants is not reached. Enabling this will affect live forms.
Hide the "decline to participate" button on the 360 review form forDoes not allow the selected users to decline to complete the form. Enabling this will affect live forms.
Enable Add New Participants after 360 Review Evaluation starts forThis feature gives you the ability to add new participants to a 360 review after the evaluation has started. Enabling this will affect live forms.
Enable Send Back forEnables the selected users to send a form back a step. Enabling this will affect live forms.
Enable Send Reminder Email forEnables the selected users to send email reminders to raters. Enabling this will affect live forms.
Enable Remove Participants after 360 Review Evaluation starts forEnables a user to remove participants after the evaluation has started for the subject. Enabling this will affect live forms.
Enable Gap Analysis View from Detailed 360 Review ReportEnables the gap analysis report in the Detailed 360 Report. Enabling this will affect live forms.
Enable Rank View from Detailed 360 Review ReportEnables the rank view report in the Detailed 360 Report. Enabling this will affect live forms.
Show Display Options in Graphical Summary ViewEnables the option to have Display Options in the Graphical Summary View in the Detailed 360 Report. The default options available will be selected from the next setting, Default Display Options in Graphical Summary View. Still, users will be able to select any other available sections as well.

Default Display Options in Graphical Summary View:

Specifies which sections users see by default in the Graphical Summary view of the Detailed 360 Report. The options are:

  • Show descriptions
  • Show rating labels
  • Show scales
  • Show comments
  • Show rating values
  • Rank
Calculate the item ratings by sub-item ratings in Detailed 360 Review ReportCalculates the rating for an individual item including sub-items that may be included. Example: Goals can have tables with fields considered sub-fields that could play into the overall rating. Enabling this will affect live forms.
Display the ratings in Detailed 360 Review Report with the decimal places which are set in form templateDisplays the exact calculated scores, rather than rounding scores for in the Detailed 360 Report. Enabling this will affect live forms.
Display Section Ratings in Detailed 360 Review ReportSpecifies that ratings for each rated section are shown in the Detailed 360 Report. If this option is not selected, only overall ratings are shown. Enabling this will affect live forms.
Enable enhanced Detailed 360 Review Report anonymityHides categories in the comments section of the Detailed 360 review report.
Hide the email sender for 360 Review Document Complete Notification and 360 Review Document Return to Evaluation Notification. (Only applicable for Anonymous 360 Review)Maintains anonymity by hiding the name of the user who sent a 360 review form from the completed step back to the evaluation step, or when the document moves into the completed route step.
Show Signoff routing step names in Signoff StageEnabling this option will show the description of the person that is to sign the document in the Signature Stage. Enabling this will affect live forms.
Enable form title editability in Form Info pageAllows the user to edit the title of the form while working in the Form Information page

Enable Word XML ExportAllows users to export the form in XML format. Enabling this will affect live forms.
Enable 360 Add External Participants sectionRecommended when external participants are part of your process.
Hide the link to detailed 360 review report in the En Route and Completed folders until all users have completed the 360 reviewHides the Open Detailed 360 Report link report until all raters have submitted their ratings and the form is no longer in the Evaluation route step. Enabling this will affect live forms.

Disallow further invitations when a user has already been invited to rate x forms

If the maximum invitations are exceeded:

  • Don’t allow
  • Display warning and allow

Limit the number of times an individual can be requested to be a participant for a 360 Review. Configure the maximum number of times a user could be invited to be a participant for a review, then when adding participants.

The following option If the maximum invitations are exceeded: system will throw an error popup message preventing the participant from being added (Don’t allow), or will just display a warning and allow the participant to be added, regardless of exceeding the number of invitations (Display warning and allow). Enabling this will affect live forms.

Inherit Permissions through Send Copy

  • 360 Detailed Report Permission
  • 360 Drilldown Permission

Allows user who has been sent a copy of a completed form from the Send Copy button to inherit any permissions to view the Detailed 360 Report or the completed 360 Review forms. Enabling this will affect live forms.
Hide the Show Assignment buttonNo longer allows the form's originator to select sections that participants can rate. If you select this option, all participants will have to provide ratings for all sections.
Only create for users who don’t have an _form with an End Date:Choose between existing and in-progress to define whether forms can be created for users with outstanding forms. Additionally, the option Do not allow admin users to edit the date range in the Launch Form tool can be enabled to prevent admins from launching additional forms if this restriction is in place.
Show digital signatures in Document Print PreviewSpecifies that the signer’s name is shown on the printed copy of the form, as well as their role and the date when the user signed the form. Enabling this will affect live forms.
Disable Due Date ValidationDisables a validation by which the form is checked to make sure the end date is before or the same as the due date.
Disallow users from changing the Start/End/Due dateDoes not allow users to change the default start/end/due dates that have been configured for the form. Enabling this will affect live forms.
Enable form routing to previous stepAllows a form to be routed back a step. This can be used in the place of Iterative Steps. Enabling this will affect live forms.
Prevent forms from being created with overlapping date ranges (this applies to all templates with this configuration enabled).Doesn’t allow forms to be created during overlapping time periods. It applies to all templates with this configuration enabled.
Enforce permissioning for Detailed Document Search & Archive & Print ReportingWhen a user with permission to run reports and search through data searches through forms, the permissions for roles, by each step of the route map are enforced. If the user running a report searches through performance forms, but a form is in a route step where it is only visible and editable to the manager role, the user running the report will not have access to any data from that form. Enabling this will affect live forms.
Define the PDF file name formatIf the toolbar button to allow the form to be printed in PDF format is enabled, the administrator can decide from the default option Form Title and Creation ID, or customize it from the list of available variables to define the PDF file name.
