In SAP Entitlement Management, a consumption form is a document or data input form used to track the consumption of entitlements for a specific product or service. It captures information such as the type of entitlement, the quantity consumed, the user or customer consuming the entitlement, and the time of the consumption.

If an existing consumption form is already in use in an entitlement model, you can only add more fields or adjust the position of the fields, but you cannot delete the form. Also, you cannot delete predefined consumptions forms.
